What Your Handwriting May Signal About Cognitive Decline, Say Researchers

Here’s how the number of pen strokes and lettering size could convey the general health of your brain.

Writing by hand might seem effortless, but behind the scenes, the brain is putting in a lot of work. Handwriting requires a combination of movement, coordination, memory, and thinking—all working together at once. Which is why researchers say subtle changes in handwriting patterns could be a tool to help doctors easily spot cognitive decline.

Researchers in Portugal recruited 58 older adults between the ages of 62 and 99 living in care facilities. Participants were evaluated using a common test for cognitive impairment, which classified them into two groups: 20 cognitively healthy and 38 cognitively impaired.

The May 2026 study, published in Frontiers in Human Neuroscience, tested the participants’ handwriting using a digital tablet and pen. First participants were instructed to complete two tasks that demonstrated their pen control. They were first asked to draw at least 10 horizontal lines within 20 seconds. For their second task, they made at least 10 dots within the same time frame.

Once their pen control was established, participants also underwent four handwriting speed tests that included copying and dictation exercises, which included the following, respectively:

  1. Reading a sentence on a card then copying it on their tablet
  2. Hearing the same sentence dictated then copying it on their tablet
  3. Listening to a new sentence being dictated and writing it on their tablet
  4. Seeing the second sentence again on a card and copying it to their tablet

Researchers then analyzed how long it took them to write each sentence and the detailed movements of their pen (called kinematic features), which include speed, smoothness, slant, pauses, stroke count, pen pressure, and horizontal and vertical letter size.

What handwriting suggests about cognitive health

The research team found that cognitively healthy adults and adults with cognitive impairments show almost no meaningful differences when it comes to basic handwriting tasks. The groups had similar results for the line and dot drawing tests. In fact, there were no statistically significant differences for the first two handwriting test results between the groups, either. This suggests that basic motor control may stay preserved even as cognitive health declines.

However, differences emerged during the third test when a new phrase was introduced. Researchers dictated the new phrase for the participants to write down (as opposed to seeing it first on a card). The study’s senior author, Ana Rita Matias, PhD, says in a press release that dictation tasks “require the brain to do multiple things at once: listen, process language, convert sounds into written form, and coordinate movement.”

It took the group with worse cognitive health longer to write down and they used more pen strokes. Researchers also noticed that they started writing slightly later and their letters tended to be larger vertically, which may be a sign of worse motor planning and executive control. Since cognitive decline affects memory and executive function, it may explain why listening and writing simultaneously becomes much more difficult.

Researchers write in the study, “This interpretation is consistent with prior work linking executive dysfunction and working memory deficits to reduced handwriting fluency in mild cognitive impairment and early dementia.”

A new possible way to easily assess cognitive decline

Ultimately, the researchers say these findings could help shape a cost-effective, non-invasive method for the early detection of cognitive decline. The study concluded that handwriting analysis may offer a “clinically relevant approach for detecting cognitive vulnerability.”

While the findings are promising for easy, early detection, the sample size is fairly small and more research is necessary. Experts say the best ways to monitor cognitive health include regular doctor visits, prioritizing sleep and exercise, and paying attention to changes in memory and everyday functioning.
